Privacy and confidentiality.

Our privacy statement

All participant information is treated with strict confidentiality and handled in accordance with privacy requirements. Consent is obtained from the person making the enquiry or referral before any assessment or support proceeds.

To support the best possible outcome, we encourage as much relevant information as possible to be shared at the beginning of the process. A clear picture early helps reduce anxiety for the participant and allows us to plan carefully so they are supported in a calm, respectful and appropriate way. We work collaboratively with all parties involved to ensure a smooth and considered process from the start.

Consent

The referral form asks you to confirm the following before it can be submitted:

I consent to Restore and Beyond obtaining and holding personal information for the purpose of accessing services.

Where a support coordinator, organisation or family member refers on someone else’s behalf, consent is confirmed with the participant before any assessment or support begins.

Sending documents securely

The referral form on this website is not encrypted. Please do not attach clinical documents, reports or plans to it. Once a referral is acknowledged you will be given a secure way to send those.

What is collected, and why

We collect the personal information needed to prepare for your first engagement and open your file: first name and surname, date of birth, NDIS number, contact details and your contact preference, whether you have a public guardian or nominee or manage your own NDIS plan, your ethnicity and whether you require a translator, and whether your plan has funding available for our services.

It is collected so the first meeting can be prepared for properly and your participant file can be created.

How records are stored and kept

Records are kept securely and treated confidentially. In line with the NDIS Practice Standards, records are retained for a minimum of seven years. Where a record relates to a child or young person it is kept until that person turns 25, whichever period is longer, along with any further state, territory or legal requirement that applies.

Who information may be shared with

With your consent, information may be shared with the referring support coordinator, your plan manager, the NDIA, relevant health professionals, and your family where appropriate.

Information may be shared without your consent only where the law requires it, or where there is a risk of harm.

Seeing, correcting or withdrawing your information

You have the right to access, correct or update the personal information held about you, and to withdraw your consent. To do any of these, contact Restore and Beyond directly.

You do not have to wait to be asked. Your information is checked with you for accuracy, and these rights are restated, at set review points during the work.

Privacy complaints

If you have a privacy concern, please raise it with Sia Ropati directly. Email is best, because it leaves a clear record. Complaints are acknowledged within two business days, treated confidentially, investigated fairly, and answered in writing. You will never be treated unfavourably for making one.

If you are not satisfied with the outcome, you can take it further. For NDIS matters, contact the NDIS Quality and Safeguards Commission on 1800 035 544, and a complaint there can be made anonymously. For privacy matters specifically, contact the Office of the Australian Information Commissioner.

Cookies and analytics

This website uses no cookies, no analytics and no third-party tracking. Nothing you do here is recorded or passed to anyone else. The only information we receive is what you choose to type into a form and send.
