The process

How to refer

Four steps from first contact to a report or a first coaching session.

  1. Send the referral form

    Participant details, plan management type and the service you are requesting. Add your own details as the support coordinator so there is a point of contact.

  2. Consent is confirmed

    Consent is obtained from the person making the enquiry or referral before any assessment or support proceeds.

  3. The initial meeting

    In person wherever possible, at home or at an agreed location: meeting face to face makes it easier to build trust and to understand the person in their own setting. Telehealth is available where distance or circumstances make that difficult, including regional Queensland. The fuller history is gathered here rather than through paperwork up front.

  4. Report or ongoing coaching

    For an assessment, a written report follows within 2–4 weeks of initial contact. For recovery coaching, sessions usually run weekly or fortnightly with a recovery plan developed in the first 4–8 weeks.

Language

Samoan is spoken. Other Pacific languages can be worked with, and interpreters arranged. Note on the form if a translator is required.

Fees

Charged in line with the current NDIS Pricing Arrangements. Functional cleaning is funded under Core Supports, Assistance with Daily Living, or booked privately at $180 per hour, from $500 per booking. Assessments and recovery coaching are both charged at the NDIS price limit for their support type. Report-writing time is billed separately from face-to-face time. Reasonable travel may be claimed under the NDIS provider-travel rules where the plan and service agreement allow.

How information is handled

All participant information is treated with strict confidentiality and handled in accordance with privacy requirements. Consent is obtained from the person making the enquiry or referral before any assessment or support proceeds.

To support the best possible outcome, we encourage as much relevant information as possible to be shared at the beginning of the process. A clear picture early helps reduce anxiety for the participant and allows us to plan carefully so they are supported in a calm, respectful and appropriate way. We work collaboratively with all parties involved to ensure a smooth and considered process from the start.
